Medication At School
  • Some students may need to take medication prescribed by a physician during the school day in order to be able to attend school and/or participate in the educational program. 
  • With written permission from both the student's physician and the parent/guardian, medications can be administered by the School Nurse or designated school personnel
  • This form is needed for all medications (prescription and over the counter) that need to be taken at school. 
  •  All medications need to be kept with the school nurse and will be distributed to student per the health professional's written order. 
  • The only medications allowed by state law to be carried by the student are rescue inhalers, auto-injectable epinephrine and insulin--but even these need to have a Medication At School form filled out and kept on file in the Health Office.

PE Exemption
There are 2 types of exemptions for PE:
•A temporary exemption is good for one school year and may be renewed annually.  This is helpful for students that might be be recovering from a temporary illness/injury but plan to do activities in the future.  During the year of the exemption, however, the student is not allowed to participate in any school sponsored sports activities/teams.
•A permanent exemption is for those students with chronic illnesses, injuries or other medical conditions where it is known that no physical activity will be appropriate. These students are also restricted from participating in any other school sponsored sports activities/teams. Please note that once a permanent PE exemption form is signed, it cannot be reversed.
